     Sheridan Memorial Clinic parking is available on the corner of Highland Avenue and Maurice Street. Use North Entrance door "C1".

     Entrance for the VA Clinic, Outreach Clinic to include DME, Home Care/VNS, Telemed and Visiting Providers is on West Laurel Avenue, Entrance Door "B4"

Caring for Our Community

Our providers can treat acute medical illnesses and injuries as well as long-term care.
We have onsite Radiology, Therapy and Laboratory services.
Insurance and Medicaid Co-Pays are due at time of service.
Self-pay visits are asked to pay for visit at time of service.

Passport Provider:

Sheridan Memorial is a Medicaid Passport Provider. If you need to switch your passport provider, Sheridan Memorial Clinic staff can assist you with that.
