Long Term Care - Care Coordinator
Description: Responsible for assisting the Director of Nursing with Resident care coordination to include admissions, training, care planning, progress visits, minimum data set (MDS) reporting.
Hours: Part-time; 30 hours per week (0.75 FTE); Benefit Eligible.
Requirements: Must possess a RN license in the state of Montana or be able to obtain. Must obtain MDS Certification within six months of hire. Must be able to lift up to fifty (50) lbs. and practice safe work habits.
